93 Previa Gas Spills

We bought a 93 Previa earlier this year and have had some problems that are leated to the gas cap and fuel nozzle area. When we first got the van we would tighten the gas cap after fueling, but the next time for sueling, the cap would be almost impossible to open, so we decided to just screw it on until we felt a little resistance.
Now, when we fuel up (not always a full tank, BTW), about an hour after we park it in the garage, we smell gas and find a large puddle around the driver-side rear wheel, like it has been spilling out of the filler nozzle.
Any ideas?
